I asked the same question on dpreview and someone suggested that AF-C is only set in the "I menu." Since I didn't remember how I set it up, I looked at the "i menu." It turns out that when I hit the button to bring up the menu, the initial image under auto focus mode shows the letter M. When you hit okay, the window changes and you see the choices from left to right of AF-A, AF-S, AF-C and MF. The AF-C is highlighted in yellow. When I hit the okay button it reverts to the initial "i menu." Is this something to be concerned about?
To be sure you understand, in custom settings I have a1 AF-C priority selection set as release, in f2 AE-L and AF-L is set as AF-ON, on c1 the shutter-release button AE-L is set to OFF.

Make sure out of focus release is set to enable. It is the next step after selecting AF-ON only.
