I didn't have the proof of purchase and simply explained that to them. They wanted to know where I bought it, but I couldn't recall, and told them as much. They wanted photos of the front and the back of the card (preferably that will reveal the serial number on the back), which I provided. Of course, they wanted an explanation of the issue (mine would not format in either the camera nor both of my computers).
All in all, it wasn't a bad experience and they were very professional about the whole thing.

Curiosity killed the cat, I had to look, so with my pocket knife I opened the card all the way. Would have a hard time now proving it just fell apart.
