I don't like a large lens still attached as a carry around rig.
I'll use a smaller lens as a ready setup, and I;ll pack my bigger lens in a sock.
Yeah, I got big feet. Also.

For those who ask, I have a handle like this and it says FilmCity on the back. I like it for hand held carry. lots of adjustment for attaching the tripod mount via a 1/4x29 screw.
PS: for when I use the carry handle (FilmCity) I still keep a wrist strap on in case of any little screw or thread failure - belts and suspenders type guy!

Depends on what I'm doing. If I'm outside, which is where most of my shooting takes place, and if I have to travel any appreciable distance, I carry every thing inside a compartmentalized backpack, with the tripod strap around the backpack vertically. That way, I can unload, get tools, and be ready to shoot in about a minute. And it really gets rid of any strain from hand carrying, as well as leaving my hands free to negotiate barriers.

The company name is filmcity, they specialize in video cages for video work. It may take some effort to find my exact handle.
I use my 200-500 zoom on my D850 frequently. How I carry it depends upon where I am and where I'm going.
For level ground, I will carry the rig by the tripod foot of the 200-500 lens. If I am going off trail, the camera/lens goes into my backpack so I have both hands free to assist in any climb while not risking my equipment. And I have a larger backpack for 2 bodies and 3-5 lenses as well as a 1-2 extra lens smaller backpack chosen by what I'm shooting and where I'm going.
This attaches to the hot shoe and seems designed to hold and carry other accessories. I don't know if that camera shoe is designed to and strong enough to carry a camera plus heavy lens attached.

I also use a Black Rapid sling with my Canon R5 and 100-400 with 1.4X and adapter.
I connect it to my RRS base plate with a RRS quick release. That way I can get it off my sling and onto a tripod in seconds. The camera rides nicely on my hip and is easy to walk with - and I can bring it to my eye in a second.
Black Rapid now sells the sling with a RRS quick release. You may have to hunt for it on their website. I had to cut my old Black Rapid and resew it with the RRS quick release.
I like my cotton carrier vest. Carried camera and my 150-600 zoom tight to my chest with the safety strap holding the end tight to me also
An arca swiss plate from RRS with the qd connection is the easiest & cleanest. RRS use to make the lens/tripod foot for the Nikon 200-500. Don't know if they still do & if it has the QD connection on it.
I attached a benro arca swiss plate to the bottom of the lens foot with a swivel half moon type screw & use a caribinar with the Peak Design Anchor set-up with my Nikon strap. The plate , screw &, Anchor set up was purchased from B&H. This with the camera strap you probably already own is the cheapest & probably the quickest way to geterr done.
