With a simple answer.
Who is at fault in Alex Baldwin's firearm incident
I am talking about adults.
I guess I should give my answer.
The only person at fault is the person that pulled the trigger!!! No one else!!!
All guns Are loaded until YOU clear them!!!
If you don't know how to clear a weapon LEAVE IT ALONE!!!

The person who loaded the gun.
My question is: why are real guns used? A fake gun with sound effects should be just as good.

The shooter. I have been around guns my entire life and spent over twenty years in the military. Anyone handing you a gun must check and clear it; however, the receiver has a responsibility to check and make sure it is clear or has the correct ammunition. Ultimately the shooter has full responsibility.

His job is to make sure the gun is properly loaded before handing it off to an actor, any actor, for a scene.

Trust issues?
Some have to have the magazines unloaded to see if the rounds are real or not.
Are all actors capable of doing that? Are they capable of reloading the magazine?

Four cardinal rules of gun safety. Baldwin failed to follow any of them. Plenty of negligence to go around, but the buck stops with the shooter. And the director, also Baldwin.

Probably not, but they should be or not take the part.
If you don't know how to handle the TOOL don't mess with it !!!

Almost all of them on the set including Baldwin. A gun is not a toy (or prop), always treat a gun as if loaded, never point a gun at anything you don't intend to kill. There's plenty of blame to go around on this and in my opinion the authorities should make sure of it.

The shooter is ultimately responsible, always check your weapon. Even at a range - check, load, shoot, check.

So, how many actors, do you think, are fully trained in the use of firearms? All off them who use them in scenes?

Were they doing a dry run to see how it looked in the camera before committing the scene?

I can't see where there would EVER be a need to have live rounds on a movie set. None at all.
If they need loose rounds with bullets for effect, they can mount the bullet in a powder-less cartridge, with a dummy primer.
