My guess. It's a registration card for passenger on ocean-going vessel for identification if something goes wrong, really wrong.

I think the GPO stands for Government Printing Office. The numbers following the GPO do not respond to a search of the GPO such that the card is probably more than 60 years old. Just my guess.
