PS spot healing brush has become the patch tool.

Your welcome Ken. I'm old school and liked the old interface on the start. This was one of the first options I fixed.

I unchecked all of the boxes. Should I have left some checked?

Welcome philo, any of the other boxes you unchecked are really up to your preferences. The screenshot I posted addressed the issue you specifically asked about.

Click on the little arrow in the right corner of the healing brush icon on the tool bar. It will open an option box, click on healing tool. You are done.

One other thing that has happen is that the icon in the tool bar has changed. The spot and healing icon was a brush and now it is something else.

I run CS6. The spot healing brush should be an oval with a band aid. See below. Also see the option when you click the small arrow in the icon.
