Some people never receive compliments so when someone compliments them they don't know how to react. Years ago I was promoted to Personnel director of my company. I made it a habit of greeting our employees each day. One older Lady accused me of flirting with her. When asked why she said it was because I always greeted her each day. I then asked her if she thought I was flirting with the male employees too. I served in similar gobs for more than 30 years with very few problems.

Yes, telling her “she seems to be letting herself go” will put a smile on a woman’s face every time.
That's better than what they tell me when they see my old pictures.... "You USED TO BE good looking."
A woman once held a door open for me,....I commented,..ohhh reverse chivalry,....thank you,....we both laughed.

Had a sister-in-law years ago that was looking at old pictures of my brother and me when we were young. She said "Oh my gosh, I thought MY family was ugly!!!"
As I am a woman, I can tell you that you made two mistakes because on the one hand, you insulted her and then complimented her, twice.
For example, you are pushing 80 but you are still attracitve.
Strike one.
Example two, just because a woman does not wear make up as I don't because I feel it is a waste of time, that does not mean that we are letting ourselves go.
Just saying that she looked great without all the other stuff would have made her day.
This is not criticism, just probably a perspective on why she got upset.
Same thing goes for never talking about weight, either losing or gaining.
