When I was in Iceland and a different time in Italy, I took my big rolling Tamrac! I have a photo of it in Iceland with grappel falling...the case worked fantastically! Here, during a snowy day, I just dragged it through the snow up/down hills. That old Tamrac has stood the test of time.
So perhaps a rolling bag will work for you? I am a Senior citizen with the usual aches. For me, this solution works best.

Normal rolling bags have very small wheels - great for airports, not so good anywhere else. I have been looking at beach carts which have large wheels which I could attach a camera bag to. I thought years ago I saw someone with the same issue who used some specialized cart with a seat but I can’t find it in the archives.
