What program do you use?
I did it in PhotoShop, it could also be done in PhotoShop Elements. First I enlarged the canvas area of the photo of the birthday girl and then with the clone stamp I extended it on the sides. Then I put the grandparents photos on their own layers on top of the girl's photo; lowered the opacity of their pix, so I could see where I wanted them behind her. Added a mask & erased where they were in front of her dress to make it look like they were behind it.
If you have PS or PSE & would like the merged picture in a psd format so you can see the layers let me know & I'll email them it to you.
