My covid shot was arranged by the school district, and we had to go to a shared site about 50 miles away to get it. I remember them giving me a card, but had no recollection at all of it after that... I searched the car, all my jacket pockets, the nooks and crannies in my wallet where my medical certificates hide out. Nope. Not a sign.
At a pre-school-year meeting, I spoke to the head of transportation, and asked if she could give any help in finding out about it. Within a day or so, she sent me a phone number of someone in the health department, who subsequently gave me an email address for the state government people who do that sort of thing.
She sent me a form, which I had to fill out and send back, along with a photo of my drivers license. It said on the form that it could take 6 to 8 weeks to get anything.
About 2 weeks later, I get this certificate through the mail. It isn't the same as the original (ask me how I know...) but it is a legal document.
Less than 3 hours later, I found the original... It was under the phone charger, and I am sure that I must have had a good reason for putting it there. Oh well...
Back to colonoscopy prep, day 2 *sigh*
