There are so many categories of birds and locations, that would be hard to answer!
Thank you every time I take a picture of a bird 95% of them are out of focus

I enjoy shooting birds on golf courses, so all of my efforts are hand held. I use a Sony A6500 and the 100-400mm image stabilized lens. For long focal lengths you need high shutter speed, I usually shoot 1/1000 to 1/2000 range, at 5.6, ISO auto. On my Sony equipment I use wide focus area, continuous focus. I improved by practicing a steady posture and stance, resting the lens on something whenever possible, shooting silent shutter, low continuous drive (about 3 fps I believe). Most problems I have had with unsharp images reflect camera movement (it is easy to get excited at moment of exposure and induce camera movement!) and some missed focus is just part of the game...I have not attempted birds in flight, that is beyond my level of talent.

It depends on so much. Available light being the most important factor. Focal length and other factors also come into play.
Depends on the situation and lens. If I am using a really long lens (say 600mm) I prefer a tripod. Under 400mm I am comfortable hand held.
