Looks like overhead ceiling light and a direct low power fill flash.
The shadows and catchlights would seem to indicate a broad light source at about her 12 0'clock and high. What it is is immaterial - window, soft box, umbrella, small nuclear device. It's light.
Then there is what looks like a small light source in they eye at 5:25 - maybe 5:23. It is just a fill because it makes no shadows on the face, but probably just fills the shadow of that main light. Whatever, looks nice.
The shot was done by bouncing the light off the ceiling. The light source was a old, very bright Sunpak 544 handle flash. I laid it on the sofa beside me and pointed it at a vaulted area in the ceiling that accommodated a tall window. It acted like a overhead umbrella. There was no window light used. The lower catchlight is the flash. The upper reflection is from one of the three large curio cabinets in the room.
