Almost always, exceptions are inside only sometimes, using a CPL, lens shade gets in the way.

Lens hoods purpose in life is not to protect the lens, rather to prevent sun flares. Many use the lens flares as part of their photo, we see this often in movies... sort of a fad thing I guess. I minimize what I carry, but I do have a rubber 58mm that came with one used lens, never used it.
Devised one for my favorite 24/7 camera, Sony HX-50. An eye cup for eye-view just fits the 37mm snout. Why did I do it? Just for the fun of it, I guess... not because there was a need... another solution looking for a need ... humm that describes many of the things sold for photography!
I find it hard to use when I have to rotate my polarizer filter. And it gets in the way when I zoom out and use the wide angle. I have tried many hoods but none seem to work for me.

You can buy lens hoods that screw on.
These can be better for using with polarisers.
If your lens hood gets in the way when you zoom out to wide angle you are using the wrong lens hood for that lens, or using it incorrectly.
All lenses have a recommended hood to be used with them.
Using a different hood and then complaining about it is silly.

All the time. It never comes off.
I only take the hood off with some zoom lenses when I am at the very widest.
The hood that comes with a prime lens should never be a problem.

I remove them........but only when I put the camera back in the case.
Definitely a problem when using a polarizer, which, in my case, is on the camera a large percentage of the time. There may be hoods that allow easy manipulation of a polarizer, but the ones made for my lenses do not.

Which begs the question: Are you using the hood designed for your lens?
None of the lenshoods I have interferes with taking photos - at any zoom, wide or narrow angle.
As to the polarizer filter, yes, that could be a problem, although at least one of the lenshoods I have, has a sliding "door" on the side and I can reach the polarizing filter without taking the hood off.
