Would anyone know where I might obtain a standard model form?

Google "Standard Model Release Form"

You will receive many hits. Some for purchase, some are free, and some allow you to customize.

DS, one fotog I know has one on the back of his card. If your interested in something like that, I'll try to find his card and tell you what he has on it.

A good PDF version with fill-in fields works on any phone with Adobe Reader as well, and then can also work on computers too!!

Now just need a PDF reader that allows you to add actual signatures from a touch screen!!

I copied and filed the shortened version. By the way, can you tell me in a nutshell what the "seal" box is for? I didn't feel like wading through the site maze to try and find it.

Bought "Business and Legal Forms for Photographers by Tad Crawford", comes with a CD of all forms downloadable and customizable to the computer. Excellent source for all forms.

There is an App available called Easy release for the I Phone that allows for data and signatures.

At a time when most people could not read, official documents were signed and/or sealed with drops of wax next to a signature or to seal closed a folded document. An official stamp or ring was pressed into the hot wax to attest to the document's authenticity.

Today, when required, we use mechanical stampers to emboss a pattern (seal) into papers or small red stickers to represent seals or even just one's initials.
