FWIW I have used SD cards since they first came out. I am meticulous about powering down or "ejecting" cards from a port. I have only had three fail. The first was a small card some years ago but the other two were in the last two months.
These were both 8 GB LX Series Patriot cards. They were bought at different times and appear to be different batch numbers. The first was replaced but following the failure of the second I'm not about to use these any more so I haven't bothered with a replacement.
I would be cautious about using these for anything that matters.

Yes, I too have read many times, before inserting your SD or SDHC, SDXC, lexar, Transend, PNY, etc. card into any device, Camera, photo viewer, external hard drive, etc. SHUT OFF POWER. This is very important to the integrity of the memory card.
