I've only seen that happen with film, with two shots sharing the same frame due to the film not advancing properly. Very strange. Must be a camera processing glitch. I'm surprised that the file is still readable after that mangle!
I don't know what happened either, but it would be a good idea to thoroughly test your camera before going to shoot anything important. Check your SD or CF card to ensure it's not corrupted.
