Looking for low light shooting advice

With your D500 advancing the ISO to what you need and then using Topaz Photo AI has worked well for me

I have a D500. One thing to check are the a1: AF-C Priority Selection a2: AF-S Priority Selection. Set both of those to Focus. If they are set to Release you can take the photo even if your subject is out of focus. If those are set to Focus, the subject must be in focus before you can release the shutter.

Your flash may have had no effect if you were too far away from the main subject. What was the camera to subject distance?

I don't understand, why would you use that high an ISO, esp if you are using a flash.
It sounds like you are getting the focus on the BG, and not on the subject.
I think that correcting these will greatly improve your photos.
You could also use Topaz Photo AI to improve the noise and sharpness.
