You have to wait a second or two once you get the image on your screen until a little yellow box shows up under QR code. Then press the shutter and it should work.

If your phone's (or Tablet's) camera doesn't support QR codes, there are separate apps (like "QR & Barcode Reader") that will do it.
The oldest iPhone I have is a 6s+, It works with the native "Camera" app as do newer iPhones after that model.
