When he said he thought the price is too high you should have said that's what you think it's worth and offer him to return. But for now just forget it. What's done is done.

jk, looking back at your posting history, IMHO, I think, for your own peace of mind, you might want to keep your photography and the hospital and its employees totally separate. It seems like when they've mixed, they've caused you much concern.

I believe you are the one at fault here and he owes you no apology or whatever you are looking for. The price should have been determined prior to the customization, etc. His specialty is a doctor, yours should have been pricing your work beforehand. Out of ignorance he may have well thought half your price was a fair price. If you didn't know what to charge you could have gotten back to him. And to accept $50 less (15%ish) simply because of no change was silly. A simple "you still owe me $50" or "give me $400, will give you change tomorrow" was all that was needed.

I learned long ago when someone says "its not about the money" it is always about the money. Learn your lesson, move on, life is short.

I have met and dealt many people him in my 60+ years as a Mechanic, after getting shafted a couple of times I learned not to deal with people like that.
While rebuilding a transmission I once ask a Professional why his time was worth three times my wages, he told me I HAVE A DEGREE , I just smiled and politely told him to put his transmission back together, locked my tool box and left work early!! I may be wrong but to expect common decency among professionals is not hard to imagine, or at least I thought so until your response.

I never said I thought it was a masterpiece. I didn’t ask him to buy it. He asked me to make it for him and to charge whatever “I” thought it was worth. I suppose it’s much like going to the doctors office. When was the last time you were happy with a medical bill?

If you intend to continue selling your work, even as a part-time endeavor- which is a good thing to do since you have skill and talent, make it a policy to conduct business in writing.

You don't need a degree in business administration or become a lawyer. Draw up a simple contract with specifications of the image, print size, and finish, and set the agreed price.

If the print has to be produced, for example, if it was chosen from a proof or another print, secure a deposit, at least to cover your costs.

Also, consider, it is not always easy dealing with family or coworkers in your workplace- too much politics and assumed leverage by unappreciative or somewhat unethical folks.

This doctor, from what you say, seems like a real piece of work! First, he agrees on your price, then says he does not have the money, then offers you less and gives you a higher amount, and asks for change. Not good.

I understand that you are a health professional but nowadays, with high costs of living, many folks have more than one job or have a part-time business. When you are approached by a potential "customer", tell them that you do your art as a business venture and present your prices and your requirement for a contract.

Don't feel bad or beat yourself up. This like this happens in every business.
