I'll guess this is in a cataloger.
Usually due to the file being renamed or moved via the operating system, like Windows Explorer.
Catalogers have no idea what may be done to files when not done within the cataloger.

When I loose a photo in Lightroom I look at the information that comes up when I choose “find photo”. I then copy the name of the photo that Lightroom is looking for. Next I go to my Mac computer and open up a find window. I paste in that window the name of the photo. There may be more than one photo with the same name that comes up but you should be able to see your photo if you haven’t changed it’s name.
