Looking for a model release app - phone based

I'm looking for recommendations on a model release app that I can use on my Samsung cell phone.
Simple for me to use, and understandable for the signer.

Find a paper one that you like, or better yet, consult with an attorney that will give you what you need that will be specific to the venue you are shooting in. Generic ones may not be adequate, giving you legal exposure. If you go the app route, make sure you can make a backup and a paper copy of it. The last thing you need is to lose your releases.

You can probably find a 'short form' that will fit on half of an 8-1/2x11 paper.
Print a bunch and keep in your camera bag, maybe in a 6x9 envelope.

Gene pointed you in the right direction...
Please keep in mind that a Model Release actually is governed by the Civil Code in the Jurisdiction in which the assignment took place... In order for it to be enforced successfully you would be best advised to seek legal council from an Attorney licensed to practice in the Jurisdiction of merit. Sort of what Gene tried to share with you.

Please realize that you'll need to be able to explain to the "Talent" what and why he/she has to sign this.

btw, dandev do you understand the concept of "Emancipated Minor Status " if not you need to come up the curve here if you want to work with Agency Models especially in NYC (and other major fashion markets).

That said without a (legally) signed release you'll unlikely be able to submit imagery with identifiable individuals for publication... albeit there are exceptions.

So dandev a model release is a contract signed and agreed upon by a photographer and a model before a model shoot is carried out. The purpose of this is to provide legal protections for models, photographers and anyone else involved in the licensing of the product.

Bottom Line: Any use of photos that may be considered ‘commercial’ in nature in a Court of Law should have a model release. Again there are exceptions.

I've collaborated with quite a few "Emancipated Minors" who indeed signed a Model Release (and were legally capable of same) so I could submit our imagery for publication. This is common for fashion and beauty editorials...

You seriously need to seek legal consel here... UHH is not a wise option for the aforementioned.
