Mine was a 'user errror'... went from the Nikkormat to an F2 camera, and never looked back, and never repeated the error.

When disembarking from the tour bus that took us from the cruise port at Valparaiso to Santiago airport, the tour guide jokingly told passengers to make sure they had all their belongings because anything left behind was going to go in his garage sale. My camera was stowed under the bus seat, and I got off without it. (my dumb mistake)
When I realized that I rushed out to find the tour bus was gone, of course. The cruise line had contracted with a service to help passengers from the cruise ship get on their various flights back home. One of them found out what tour bus I had been on and that it had been returned to the company garage some 20 miles away.
Fortunately I had several hours before my flight home was scheduled to depart. The young man who was helping me phoned the garage and they found my camera. He called a personal friend who drove to the garage, picked up the camera and brought it to the airport! He would not take the $40 I offered and told me, instead, to give it to his friend who had retrieved the camera from the garage.
Resetting a camera to factory defaults and forgetting that the default was JPG and not Raw.
Shot granddaughters gym meet. I use BBF and servo focus mode on a Canon 80D. Forgot that previously, for some reason, I changed focus to one shot. So I’m holding the back button and shooting bursts and whatever of the meet. And the only focus point I had was when I first pushed the back button. Needless to say, 80% of my pics had to be deleted. Shame on me.
Not removing the lens cap before trying to frame the perfect shot...
Not checking the mode before I start shooting. Makes me feel like an idiot. Always time to check your settings after a bad shot, but never before. LOL

I was in Alaska and drove to Fairbank for a chance to see a moose with a rack. As I got to the pipeline I got my chance. He was standing in the middle of the road and I reached for my camera only to find that it had a 35 mm lens attached. While I rushed to switch lenses he walked away. Never got a second chance. I still have the slide but have to point out the moose because it is so difficult to see.
As a high school yearbook photographer, I often failed to ensure that my film caught the sprockets of my Leica IIIf. I would spend the afternoon creating art only to find that the film never advanced.

My favorite dumb mistake is setting a shutter timer then NOT immediately resetting it back to normal mode. Invariably next shot or next time I use the camera, I'm wondering what the heck is wrong with my camera and furiously stabbing at the shutter button then click. DOH!!! And having missed the decisive moment, I then set about resetting the shutter to normal mode.

This happens to me way more often then i'd care to admit.
My newest grand daughter lives almost 6 hours away. I planned a weekend trip to do newborn photography. I charged batteries…and meticulously packed props and camera gear prior to leaving home checking that I had brought everything I needed for planned shots. As I was getting off the exit after a long drive from home…I realized that in my obsession to include all my gear for the trip…I forgot my suitcase with all my clothing! Got the newborn shots…
