They flew her in a helicopter, lowered her down, the backed off took the shot, then called an ambulance to pick up the body after she fell.

To early for Adobe, but I'd guess, like others, dark room magic. Layered negs?

Looks like a composite image to me. The shadow cast by the lady do not match with the shadows of the buildings. Some text are incorrect in its projection and the lightbulbs in the middle row of the sign would also make that pose impossible.

The pic has to be cut and paste, collage, etc.
Although she appears centered optically, she is way past center left of the column with her (bottom) ballast.

Cherry picker or crane; two negatives; composite cut and paste, dodge and burn; copy photo of composite; final print with touch ups.
