I had the same problem. Called Apple support and the tech's first question was: "Are you using the small, square, 5W plug sitting on your 110v power source?" I was. He recommended I quit using those as their experience was either failure or slower and slower iPad battery recharging. I purchased a 12W Apple plug (it is larger and rectangular) and I've had no problems since. Also, the charging process is much shorter than with the 5W.
