Of all of these, it’s “No problem” in response to “Thank you” that irritates me the most. This implies to me that if it had been a problem, the service (or whatever) would not have been performed.

How about when a sentence is started with "I mean" or other times when "I mean" is just used to take up time as they think of what they want to say.
