What I carry depends completely on intentions and location(s) as well as length of shoot. Extra battery and a memory card wallet go almost everywhere. I prefer a lens brush. On a paid job, two of almost everything. Best of luck.
A small tool kit
A spare body
An extra lens

I carry all of those, too.
But, let's see, I also always carry:
(1) Light meter
(2) Set of macro extension tubes
(3) Polarizing filter
And some other things.
Yes, I almost always forget the polarizer-will try to do better.
When I'm out, a 28, a 50, and an 85 (or 90 for my rangefinder).
Before I leave, I charge my batter(ies) and check my SD card for space.

A gun.
This is a sad answer. Such anger.
...Cam
I purposely made the question broad. I am reading every reply and greatly enjoying the variety of answers. And I have learned a few things.
2 battery’s, 2 cards, cleaning cloth, gun, extra ammo magazine, large pocket knife, flashlight, cell phone, glasses, wallet,

First, last, and a case for one day outing. Sometimes a polarizer. Rarely a tripod.
But for multi days add lenses and battery charger, and maybe flash.
Never extra memory.

I always have at least 2 cameras, always have extra batteries, memory cards, extra lenses, filters, just the usual stuff!
Angle finder - thats a new one to me. What does it look like and use it for what.

It depends on what and where I'm shooting.
Sometimes just the camera and spare card wallet and battery.
Sometimes the bag, if I take it, stays in the car.
Sometimes a bag for the whole outing.
Always in the bag: a 50mm; filters/wrenches; angle finder; lens cleaner; remote release; note pad/pen; spare batteries; card wallet; & probably some other stuff. (I have a red/white headband light, but that only gets packed if I'm going out at night.)
I have several to go kits ready to go at a minutes notice depending on what type of shoot it will be. All of which include 1 of 3 camera bodies and 1 of 20 lenses, often a tripod and lighting.
Of course, always the neccessary accessories to keep everything clean and working properly, which are always in my main bag and/or backpack

Battery
Memory card
Passport color checker
(This for sure when shooting off cam and strobe lighting)
