Exactly! Yes, I carry the original 3 items when I'm shooting within a short drive to my house. There's a whole lot more when I'm on a long trip.
All three, but definitely extra batteries that all have a full charge. I made that mistake one time, and it taught me a lesson I have not forgotten!

I don't use cell phone. I never carry anything spare for photography (as it's a hobby).
Those are the first three. I always add a CP filter in case of glass or water, a small thermos bottle of water, and some sort of multitool like the Leatherman Wave. Of course the hex wrench lives in the tripod bag.
BTW I once saw a guy drop his expensive camera in a river while shooting from a bridge. I always USE a neck strap or wrist strap. I say use because some folks think it looks cool to have a neck strap attached but not use it.
Don’t we first have to agree what Camera and Shoot mean... Does camera include lens or lenses...maybe. Does shoot include everything I take to a race or trackside at the railroad? Or is it just what I pull out to actually shoot with... Pretty broad topic here and answers will be equally broad.

I don't need spare battery nor memory card for a 1 day shoot. My battery and memory card are good for at least 600 shots of uncompressed RAW+JPEG. I never shoot more than that in a day.
If more than one day, at night I would transfer the files to my surface pro and charge the battery.
If any of them including camera and or lens fails? I quit shooting, go home, arrange for repair. My gear well being is much more important than the images I took.
For me, it all depends. I usually take far more than I use, especially for short trips. If I leave the truck, it mostly depends on the weather and how long. One thing I often take that hasn't been mentioned is a compass (I've been seriously lost in the dead of winter, in a snow storm).

The question, was not complete. When I go out for a day's shooting in the woods or when I am to shoot a few things. That is I will carry a back pack of stuff or just the camera.
