Per B and H, it is "SIGMA 150-600MM CNTM CAN LENS W/MOUNT CNVRT SKU# Sl1506005CCK"
On the Sigma Warranty Card, I see AF 150-600MM F/5-6.3 DG OS HSM (C) F/CANON -- Serial Number, etc.
