When I edit photos in Photoshop and bring them back into Lightroom by saving they come back with seemingly randowm exposure adjustments...usually large increases.

Is there some setting or preset I am missing?

The prescribed method to take the photo back to Lightroom, when finished with your editing in PS, is simply File|Save, then File|Close.

Is this your usual method, or are you doing something differently?

I go from LR to PS and back all the time and have not ever experienced what you're describing. Perhaps you could give us an example. The one thing I would recommend though, is to ensure that you're us using the the same colorspace in each application i.e., sRGB or Adobe RGB etc.

My first question is when you send the file to PS are you sending a copy with LR adjustments?
