djkermode and MTshooter have it right - some young 'uns with a .177 pellet gun or a .22 put some projectiles through the plate glass windows of my place of employment, a refurbished supermarket from the 1970's. There are three holes, and the square you see is tape to stop cold air coming in.

I thought it was a reflection off a petrol cap on a car.
How do you explain the very well defined circular part with reflections around the hole?

The sides of the hole are sloped, so the glass is a concave lens; those are not reflections, but the wall of the entry behind the glass - those parts of the image are wavy because of the irregularities in the walls of the hole.
