Stick out your non-photographing hand to your right or left away from your body (camera). Ask the subject wearing glasses to move their head so that their nose is pointed at that hand. Then ask them to keep their head still but move their eyes back to the camera.
The non-photographing hand is the hand not holding the camera.

Yes. Ask them to take off their glasses for the photo. Most women are willing. In fact my wife won't let me take a picture of her with them on.
Scott Kelby has a section on fixing this in his Photoshop CS6 book. It is complicated and requires another photo without the glasses. (Edit: I see someone mentioned it above...they make it sound too simple.)
Other than the above you need to control the light. You might buy one of the folding 4 ft diameter scrims and have someone hold it up for you. If you use flash it has to be off at an angle so the reflection heads off to Neverland instead of into your camera.

An editorial aside:
Scott Kelby is way more knowledgeable than I in issues of photoshopping, however it seems to me that we create way more issues to fix later by not being a little more attentive to detail when capturing the image. I spend my entire day in front of a computer-- so perhaps that's my motivation.
