I would pronounce them the same. How would you do it?
I believe most people (now) would pronounce it "two" as it is a written representation of a count of two. Just like "2" is a written representation.
BUT, maybe the Romans when describing multiple units would say "I have eye-eye pets".... Anyone know the Roman spoken word for "II"? Maybe the pronunciation they used was different that what we (would) use.

Uh, the type of numerals it is written in don't change it, it is still "two". Though when used at the end of someone's name they are usually verbalized as second, third etc.
