If you will notice the ribbon display above the medals he wears you will see a dark purple background on the ribbon in the upper right (it almost blends in with the background color of his uniform). On that background there are 2 small gold stars. Only one Purple Heart medal is issued, the 2 additional gold stars represent the fact that he was awarded the Purple Heart 3 times.
