That is true but in the case of the church he may have come close to the upper limit in the windows. This is a case of exposing for the highlights and developing for (recovering) the highlights.

I will put together an example on this in a separate thread. I'll post a link to it here when it's ready.

Yep, bright windows and relatively dark rooms can push DR to the max.

It sure does.

I posted a demonstration in The Effect of ISO on a Scene With a High Dynamic Range (DR).

I exaggerated the ISO settings a little to explain what happened to the scene in the church above.
