I bought one of these after seeing it on UHH and like it. I use mine in the "under" position because I find the extra extension allows me to steady the load against my hip for hand holding, much like a standing rifle position. Works well for me. I can carry the rig upside down.
I use a Black Rapid Curve cross body strap to carry my D500 with 200-500mm on hikes in search of wildlife. Securely connects to tripod plate on the lens.

At 79 I've been doing that for about a decade now. Not because of my shoulder pains but because the strap was always in the way and catching on things.

Same here. I also just carry by the lens collar more often as not.

I carry mine by the tri pod foot I use a Black Rapid strap and have all the attachments on the lens keeping all of that weight off of my camera
