Knew a guy who died in a forklift accident. He was driving a forklift up an incline which it normally could handle but it began to stall out. He turned sideways and it flipped over on him. His wife was working a few stories above in an office at the time, where I was working.

Well, I do think he tried to put the breaks on first but they didn't hold, so he didn't have much choice.
