I went to one of those characters when I was a kid back in rural Ireland. He was a travelling dentist and only came to town once a month. His surgery was an empty room above the Chemist's shop with just a small table for his tool bag and a hard wooden chair for the patient. No fillings - just extractions. I still have 'pockets' where the tooth used to be. Not that there weren't proper dentists in Ireland at the time - just not in our town and Mom didn't know any better. It was NOT painless!!!
Darn - I'm starting to sound like my father with his stories of walking 3 miles to school across frosty fields in his bare feet.
