Welcome back Graham. I hope the recovery is going well. At first I was going to comment on the lack of difference between the dark background and the dark clothes and the dark skin, but then I thought it's the eyes that stand out and that's all that's needed.

The eyes are really good but could you have managed to get the garlic in focus as well?

Hello R.G. I'm becoming much more able, thanks, I can walk a fair distance. It's the fatigue is my biggest problem.

I did want a sinister look to it, but on here it appears a bit darker than I wanted. Perhaps walking isn't the only thing I have to re-learn

I would guess this is a crop from a bigger image? The eyes whilst striking are probably a little too stark white for realism. If the photograph is cropped so that only the top of the garlic shows the fact that it is out of focus is not so apparent and causes less distraction allowing ones eye to rest on the man.
Just my ideas on an excellent capture. No idea about your health issues but wish you a full and speedy recovery.

A great shot, emphasizing what you wish. The compositional circle of lights reinforcing a focus on the face is a great choice.
