Doesn’t this require an adapter? Is that included?

They screw in to the end of your lens like a filter. They come with numbers for the amount of magnification and may be stacked.

When I looked at it on B&H it said it required some kind of adapter.

The adapter is for using it on a Powershot" camera. It does not need an adapter for regular DSLR lenses.

It screws on as described by others provided you are using a DSLR lens. It essentially turns a regular lens into a macro lens.

And the DSLR lens you are using accepts 58mm filters. Otherwise you will need a step-up or step-down adapter to resize to your lens size. My 18-135mm Canon lens takes a 67mm filter. I would need a step-down adapter but going from 67 to 58mm probably wouldn't be a good idea.
