Old, worn out hankie. Absolutely clean of course.
Old, worn out cloth diaper also. Double down on the absolutely clean part.

Microfiber cloths are available in bulk from a variety of sources.

Small, smooth finish ones for lens cleaning I buy them off Amazon in packs of 10 or 12 or more (example... http://www.amazon.com/Pack--Premium-Microfiber-Cleaning-Cloth/dp/B00P7IPWZ8/ref=sr_1_21?s=electronics&ie=UTF8&qid=1424546572&sr=1-21&keywords=microfiber+cloth).

My local auto parts store has larger ones (about the size of a hand towel or wash cloth, sort of a terry cloth type finish). I see these are selling on Amazon, too.

Zeiss Lens Cleaning Wipes, individually wrapped wipes sold in the Optical Department at Walmart. Grab a handful and throw them in your camera bag (or pocket) before going out for a photo shoot.

I use the Walmart lens cleaning solution that I use for my eyeglasses. I spray it on a microfiber cloth and gently clean the lens with a swirling motion starting in the center of the lens and working out to the edge.

I have found 600 grit wet and dry with a little lens cleaner works better than steel wool if the lens is really grubby ;)

Lens specific tissue from a photo retailer and lens cleaning fluid from same. Blow brush or the "rocket" blower, a clean tooth brush (gets all the dirt/dust out of cracks etc.) a very soft well washed cotton cloth and a shoe polish brush(clean) for large areas excluding the glass. (This stuff works for cleaning the body as well, many think of just glass but a complete kit cleans all of the lens and body
