ISO reverts back to over 800 after I set it to not go above 400

I am not a professional but have been asked to do several engagement and bridal shoots, which came out very well outdoors. I still am trying to learn all of the camera adjustments according to the environment and lighting. I attended a friend's daughter's wedding. It was inside a church and at night. I used various lenses, but all which were set at 2.0 or 1.8 f stop. I used my flash because of the low lighting within the church and my relatively close proximity to the bride and groom. I set my ISO early on to not go above 400 and took most of the shots in Aperature setting. When reviewing the shots, every single one showed ISO of 1600 to 3200 and of course were very grainy. Please help! I don't know what I am doing wrong. Thank goodness I was not the main photographer because I would have felt horrible to give them grainy shots. The composition, however, was awesome. They would have been fabulous shots had it not been for this.

Knowing what your camera is would go a long way towards getting you some advise. You have asked a blind question here as almost all cameras have different commands and menus.

Sorry, I have a Canon t2i. I use Canon lenses, most of which cost considerably more than the camera. I am building up my collection gradually.
