I know that I have seen It on this site a place that you can go and down load a forms for Model , Property and print releases.

I have searched the UHH for release but no luck I don't remember what the title was.

Any help would be great

I got one from somebody that linked to the NYIP. I don't remember the title of the thread but I am sure if you go to the New York Institute of Photography website you could find it on there.
