I am having a hard time getting a picture of what people are wanting if they say take a picture of something with a "design within a image" does anyone have an example or two that could be posted to help me understand that?

Look very closely at the Marine Corps emblem. Many of the disgruntled will see a buzzard sitting on a beachball with an anchor up his butt, wishing he had joined the Navy. Semper Fi!
Ok so I though it was more interesting than just taking and inserting another image into another image? I thought it was meant to capture in one image the "design within image" rather than photoshoping it in? Ok different mindset for that.
I haven't heard that term, but maybe you're referring to something like a close-up of a leaf to show its veins?
Or do you mean the "story" in an image? Tell us more about how you came to the term, and what it means to you.
it was just an photo club assignment and I guess I think they are thinking some of the photos of something abstract or such, or maybe like when you take a closeup portrait or water drop and can see the reflected image of something else in the eyes or water drop? Not sure I'll find out I guess on tuesday night what others have done with the idea.

In any of my classes, when we are given a new assignment, we are also shown as many as a hundred pics, by many photographers, as examples of what would satisfy the assignment. Then we get creative from there.
I bet the person/committee that made up the challenge knows exactly what they mean. Maybe they could share. I wish you well with the guessing game!! ;-)

There was an online article a couple of days ago about company logos that feature a design within a design. One example was the FedEx logo. Between the "E" and the "X," there is an arrow pointed forward.

Perhaps you are to look for interesting PATTERNS within the image. When you magnify an image, like magnifying the eye of a fly and seeing the amazing pattern of the multiple eyes, it's like discovering another universe. Same goes for leaves, the veins in a rock, etc. You are exploring what most people pay no attention to. It sounds like that is what is wanted here. It doesn't all have to be a magnification, however. Even larger images can be cropped to reveal very interesting patterns. It is really quite fun. Or maybe it is about composition. You should ask for clarification from whomever it was that gave you that assignment if that is what it is.
