Looks like a composite to me....either through PS, studio set up, or more likely a combination of both. I would guess its a real hand but not a real face...doll, drawing, painting...something. The two do not go together at all. Frankly, not my cup of tea.

You could probably get more critique in either the Photo Analysis or Photo Critique and Analysis section of the Forum. Since you asked, though, I would crop it above the wrist and still don't know what the "story" is here.
Hand and face do seem incongruous. I am also not a big fan of the washed out effect you used here. Top of the head is completely blown out. I would think you did this on purpose, but it is a distraction to me. I'm sure that you spent a lot of time and effort on this composition and I'm also sure that there are members on the forum who would like it very much. I afraid that I remain somewhat less than enthusiastic.
You got some good feedback. Try another one. I think you have some great ideas with your imagination.
I love this, it's different for this site. You focus is wonderful, the pose suits post processing, her eye contact with the camera is eerie and somewhat haunting.
I looked at your flckr page (cause I was super curious about your other work)and your work is top notch and was refreshing to see. I love your sense of humor that is evident in many of your samples.
