They vary in size, from 2-mm to 4-mm. This one is probably 3-mm. I struggled with blowing the detail in the whites. I threw out quite a few. this one was the only one I liked out of ten or so.

Yes.
This scale insect infests twigs and branches. The mature hermaphrodite is 5-mm long. When mature, the insect remains stationary, attaches itself to the plant by waxy secretions, and produces a white egg sac.

No. Generally, I photograph something, wonder what it is and then go research it-- or someone on here tells me what it is. ;-)
This one I am sure about-- the reference photos are an exact match

I don't believe they go all white. They get as large as 5 mm, according to what I looked up-- but I've not seen one that large. They are all over a japonica about two blocks from my house. They are considered fairly destructive-- but it's not my bush... so they live on....
