I use Noise Ninja. It is used widely in the newspaper industry I believe.
What I like about it is its ease of use after a bit of pre-setup. It has a built in color chart that you display on your screen. You then take a shot with your camera at each ISO setting and save each shot as a profile. Then any shot you import into noise Ninja, it reads the exif info for the ISO setting and automatically applies the noise reduction.
For an amateur like me, this is very helpful.

I second most the opinions expressed. I have a D200 which I love (most of the time) but high ISO/low light shooting causes greatly increased noise; I've tried most of the commercial plugins but believe Topaz DeNoise is trully the state of the art and well worth the modest investment . . .
I use Topaz De noise with Photoshop CS5, and its works well and user friendly.
