Also...most cameras provide two or three choices around the level of compression applied to JPEG files. I don't know Canon's terminology around this. But it's a choice of whether to not compress at all, to compress a little bit, a medium amount, or a lot. How are you set for that. (There's another choice for how much resolution to retain. That would be interesting to know, but it's probably of more secondary interest at this point.)
I'm thinking that one early thing to try, before we even fully understand the problem, is to move to a lower level of compression (bigger saved file) in the interim to see if the problem might go away. No concrete basis for doing that, but if it works, it would verify the problem and give you an interim fix until you decided whether to send your camera for repair...

This is digital not analog so the possibility of an image left on the card is a moot point. It's a very specific shape, a square with a notch in the bottom right corner. Must be in the conversion.
Do the details in the square look like some of them in one of the camera's menu screens? They kind of look like they might be.

The raw file does not show it. You have tried different SD cards. The problem is happening between the raw and JPG image. That implies a camera processing issue.
Definitely try Canon tech support. Please post results.

Not if it only happens in jpgs. This is clearly NOT an analog problem but a digital one.

My friend had a problem with a new camera he was getting many out of focus shots. Very low % keepers. Long story short the solution was a faster memory card. Huh? ???!!! Yep. % keepers (due to focus) went from 20% to +80%
I mention this because you say it only happens with raw+ JPEG which takes more processing time. If the processor is waiting on memory perhaps that is the result.
Might be worth a try buying a faster card. Heck it’s a good excuse to buy the latest greatest card.
