I have Lightroom and Photoshop CC and Topaz Remask. I would like to get rid of the woman in back, but keep everything else, including the background. My original file was a color dng from a Fuji X-T2, 18-135 lens.

You will have to meticulously clone in background over the woman you want to remove. Keep moving the source indicator around as you clone. It will take some time and patience.

Yeah, that's what I was afraid of. Lots of time and effort. And I guess I have to ask myself whether the shot is worth it or not.

Before cloning, you might start with a content aware fill. First, I would create a layer that totally excludes any of the two people that you want to keep in the photo. To create that layer, you can do a fairly quick content aware selection that encompasses all of the image area that contains the two people you want to leave alone. Then, select inverse, then CTRL+J to create a new layer on top of the background that excludes these two people. Then, hide the original (background/full image) layer. On the new layer, use lasso tool to completely select the woman that you want to delete. Select fill, content-aware. She is gone and replaced with content within that layer. You can then restore the background layer visibility and refine the area that content-aware replaced via the clone tool where necessary.

This was my first try.

If you used the content aware method I am both surprised and impressed that it worked so well. I will certainly add that technique to my repertoire which has been limited to cloning.

Nice job although I would have worked a little more on the woman's right sleeve and toned down the hot area between the two women. I haven't had a chance yet to try the suggested content aware method.
