I have a Liberty Mutual schedule for all my equipment. Replacement cost/no deductible. Costs about $125 per year. All my insurance is with them, so I do get a bit of a discount. They are responsive and offer superb customer service.

We have all of my camera stuff insured thru our homeowner's policy.

You do not say who you insure your home or auto with, but AAA will let you take out an insurance policy on just your camera equipment. The cost is based on your listing of camera equipment cost. I have a list of all of my camera equipment, including cards and other small items and my laptop and mobil external drive, and I change the list as needed when I buy new equipment. You will need to do a similar list and keep all equipment receipts. The AAA policy is zero deductible and I have used it twice to file a claim with no problem, once for a sensor replacement at $1000.

Our homeowners policy is with Farmers. For a small additional premium I bought a camera rider. It covers all listed equipment for replacement cost (used market replacement price) and unspecified equipment (accessories) for up to $500. It covers my equipment regardless of where I travel (outer space excluded) for all losses. To list newly acquired equipment (specified equipment), you must provide serial numbers and proof of purchase. The premium is based on the value of the listed equipment. When I buy new equipment, all I have to do is call my agent.
All my gear was stolen out of my car. My Hartford Homeowners / Car Insurance covered everything. I submitted $2400 and they paid out $2500.
If you own a home, your camera equipment (personal property) has some coverage. Probably not enough if you have lots of hi-end equipment. I have collected twice on my homeowners policy when I had lesser valued equipment. Since stepping up, I have a rider for the camera gear. I have never had a problem with a claim the two times I had a loss. She policies only cover loss. More expensive policies can also cover damage.

Rider on my home owners covers loss, theft, breakage (repair or replace) and weather damage. Not all policies cover water. No deductible. Cost is about 1% of value.

On suggestion in addition to checking you homeowner's insurance agent to see if there are riders, or additional that you may purchase of add to any current policies you may already own, check to make sure if the coverage values your gear at depreciated value, or coverage to replace your equipment based on current values.
I was a member of the PPA and one of the features that was included in the membership fee what insurance coverage. Their coverage was of course make available thru a third party insurance company.
I damaged a lens, got an estimate to have the lens repaired, filed a claim. The lens was purchased in 2006. The claim was assigned to an adjustor, the claim was emailed to to the company who promptly denied the amount of the repair estimate from Canon CPS, the estimate included a reduction on the necessary parts,$380.52 due to the age of the lens and the deductible amount the claim was denied in short fashion. The claim was submitted and subsequently denied in just a few hours.
A great many people that I know that use their photo equipment, daily as their means of income, use companies that specifically provide coverage for photographers, Hill and Usher, also Hiscox. not only provide loss and damage coverage, as well as liability coverages.
