It's a continuum from left to right. The left "guy" really is totally out of it. The figure on the right is totally in, and probably has feeling in all toes and fingers. The two in between are at intermediate states of semi-comatose, perhaps past having feelings in all extremities or activity in all brain lobes.

Your idea of this as a continuum is interesting. Also, it could be noted that everyone is doing their best to ignore this potentially-serious situation. Where are those cell phones for calling 9-1-1?
